Please click on the appropriate link below to complete the applicable Joliet Junior College Athletics Department Eligibility Forms and download the supplemental materials.

These forms are for rostered student-athletes only, not for students wishing to try-out for a team.  Those interested in joining a team should complete this recruitment form. 

 

All 2025-26 Student-Athletes MUST complete the following online forms. 

  • All forms must be submitted online. Paper copies will not be accepted.
  • Students must also submit official transcripts for high school and any college they've attended. Official transcripts are sealed or send electronically directly from the school to JJC. Forwarded or opened transcripts will not be accepted.
  • Students who have attended another college will have a Transfer Tracker sent to their prior institution.
  • Students who took a semester or more off or delayed college entry after high school will need to include this form with their Pre-Participation Questionnaire.

Student Athlete Academic Information Release 

Athletics Insurance Coverage Disclaimer  must also upload copy of Insurance Card front and back, as well as 1st Agency Insurance Form  (if applicable) 

JJC Athletics Medical Services Disclaimer and Liability Forms with copies of Privacy Notice and Liability Risk Waiver for your records 

Athletics Medical History and Important Information must also upload a copy of Physical Form completed and signed by certifying physician within the last 395 days 

Concussion Protocol  – watch your email for this important addition from Athletic Training.

If you need to upload any additional documents due to file size or need to add to your previously submitted files, you can do so on the Athletic Forms Upload Page. 


Remember: Student-athletes are not allowed to practice or participate until all participation forms are submitted, physical is approved by athletic trainer, and courses are enrolled in classes in the fall (or current) term.  New Wolves must also attend a New Student-Athlete Orientation. 

If you are not 18 years old yet, parent approval is needed to tryout for a team or participate in any sport.
